Namun, Mia kembali hidup dan menyerang Ethan. Meski sempat ditahan oleh Jack Baker, Ethan berhasil meloloskan diri dan mendapat bantuan dari Zoe, putri keluarga Baker.
Zoe memberitahu Ethan bahwa Mia, dia beserta keluarganya telah terinfeksi penyakit aneh, namun bisa disembuhkan menggunakan serum khusus. Untuk bertahan hidup dan mendapatkan serum tersebut, Ethan memanfaatkan berbagai alat dan senjata di Resident Evil 7 Free Download. Item penyembuh juga tersedia untuk memulihkan darah yang berkurang akibat serangan musuh. Selain menggunakan item, darah pemain dapat pulih dengan sendirinya karena adanya fitur Regeneration.
